Jacob Academy requires you, the parent, to provide us with the immunization records of your child along with our physical form to be signed by your child’s Pediatrician. You need to fill out the application, financial agreement, and emergency card forms provided below too.
The three latter forms should be completed before your child’s enrollment to our facility. The physical form and immunization record may follow after 2 weeks.
